Hi, is there an opt out template for refusing the surveys, or is just the opt out test letter be enough?

Our opt out letters included surveys in each one. Those letters were pinned to the Pine City for Common Sense Education group for a number of weeks. Please check that group and you’ll find the post that starts, “Fall is here” and includes a picture with backpack with student reminders of what to bring to school: pencil, paper, student opt out letter. We have three (3) letters actually.
